Your premium is set by HealthCare.gov after you request a quote on its site. It depends on the coverage you pick, where you live, your history and the carrier's own underwriting, and can be higher than any starting figure the carrier advertises.
Individual and family health plans sold through the federal Marketplace in Bronze, Silver, Gold and Platinum categories, plus Catastrophic plans for people under 30 or with an exemption. All Marketplace plans in every plan category must cover the same 10 essential health benefits, including preventive services.
